This multi-lingual assessment form was prepared to enable a school to assess the ability of students to come to school during the COVID pandemic.
This quiz was created for a small winery to help promote their offerings and direct the customer to a product recommendation based on the results of quiz.
This is a live assessment from our Data Quality offering that integrates with Google Sheets to generate a custom report that is emailed to the user. Demonstrates the ability to create 'scores' in sections and in the overall report and provide customized feedback to the user.
This quiz is a template example for a 2-day conference and allows the user to select the day and the sessions they attended and for which they wish to give feedback. I have used this for registration for a Data Quality Improvement Boot Camp.
Here is a simple template to demonstrate that you can map users from many questions to a number of possible outcomes/ending pages/offers.
